What format the final file should be? Will PSD do?

thanks for your questions.

As I understand we get a PSD file, not HTML code. So please provide it as PSD.

The Headline in the upper left should be "Social Network Umfrage".

In the text box there should be space for a headline and around 60-80 words and the button to start the survey.

Well by the way you Are looking for Only Landing page Or you need Survey Page Also ?
And is there Any problem if we Design in English i thing you may Change Language at The Time of html/css Conversion.

we only need the landing page. The survey page will look pretty much the same except that we replace the text. You do it in english of course.
