1. Would you specify the size of the cover(book)? or the size in which design should be submitted?
2. the title headline spellings are correct?
3. Should I include our name? Publishers name?

6x9 in, yes I think the spelling is correct...I would capitalize Through the Pattern of Depression. No thank you for the publisher/ your design. I don't have any details to add.

I'm sure you wouldn't want " ANTS" images/clip-art/drawings etc on the front cover ? or would you ?

The book is about personal transformation specifically around energy creation, management and transformation. Use of language at it applies to self talk and how health and focus impact this as well. creating a vision and mission statement that inspires you to take action...drawing from past experiences to gain strength, courage and fortitude. I teach the basics of meditation and visualization of your new life in support of your new self image.
