Infographic Design Brief

I need to create an infographic design that can help me explain the process of a new project that I am working on. The project is a step by step illustration on what needs to be done in order to get a stroke patient that proper wheelchair.

The graphics should walk a client through the process starting with #1 in the inpatient process and the explanation of the steps through the last step in the outpatient process. I uploaded two documents but the document with the explanation is the document to build the illustration on. This is a healthcare piece so the graphics should represent healthcare
