Caridad Center is the largest free clinic in Florida serving the uninsured working poor. We are launching a business, "Caridad Community Care", that we hope will generate revenue to help support our mission. We train certified community health workers (lay people from the community) to provide case management to people recently released from the hospital. The CHW's make home visits, link the patients to social services in the community and help them navigate their health care options. We focus on addressing the social determinants of health (Economic Stability, Education, Social and Community Context, Health and Health Care, Neighborhood and Built Environment) to keep people out of the ER. Our clients will be hospitals and insurance companies primarily. I am attaching our non profit logo (which strangely does not reflect what we do). This new logo could incorporate elements or be different. Key concepts are health, community, and savings. And all revenue supports the mission of the nonprofit to provide free care.
