What is the name of the Journal and other sub-title info to go on the cover?

What is the size of the journal? Dimensions of the cover?

Could you please give more information about the size of the book? (the design may not fit the book if we don't know the exact size). Also, what is the title (and subtitle) of the book? Authors etc?
