Word Template Design Brief

Hello,
I need a professional Bio template developed via Microsoft WORD ASAP!!!!! I would like to just be able to add the text I need to add. So resize-able text boxes. Potentially able to adjust the font. These are the things I need added it to include.
Head shot (attached)
Text Box for Name
Text Box for phone number
Text box for email
JRB Logo (attached)
Maybe a blue banner at the top or some background boxes that are blue or grey. Here are the pantone colors that our company uses.

I also attached some examples of what we have been looking at. Instead of green add the blue...instead of black maybe just be white.
