Signage Design Brief

I need to design a display that will fit the specs of the attached. Also, the design should be as similar as possible to the "FREE VISIT" card attached, but replace the single QR code with one single phone number (833-JOINTVA) and below that say "Schedule Now" with two locations "Landmark" and "Burke" with their two different QR codes below (be sure the correct QR is under the correct name!)
