Landing Page Design Brief

Our medical practice, based in Dallas, Texas, is called 'Reed Migraine Centers.' We perform the Reed Procedure on patients to treat chronic, severe head pain.

We currently have a landing page for our Facebook ads, but we'd like to improve the page's design to increase the lead conversion rate. The landing page contains a form that allows people to register for our free, on-demand informational webinar about the procedure. Either part of the registration form or all of the registration form must be above the fold.

Once a lead submits the registration form on that page, they are redirected to a different page on the site that contains the webinar video.
