Logo Design Brief

We are a Rural Health Center. A new start up. I would like a logo that I can use for employee shirts, business cards, letter head, salutations, signage, etc. I want to incorporate health care, we are a family practice so birth to senior citizen, but because we are located in a small rural farm community I would like incorporate something like blue sky, green grass, since we are in KY the bluegrass state, trees, fences. But I am open to what the designer envisions from this detail.
